Initial Intake Paperwork
Thank you for choosing Northshore Pediatric Therapy to join your child’s team!
We send our initial intake paperwork through the Fusion Web Clinic Patient Portal. Fusion Web Clinic is a secure, HIPAA-compliant, and password-protected electronic healthcare records platform. We also use AdobeSign to securely send individual forms.

Scheduling
Regular Session Schedule
Northshore Pediatric Therapy (NPT) sets consistent weekly session times for each family. These times are specifically reserved based on the agreement made by a child’s caregiver and a member of NPT’s professional team who handles scheduling. This arrangement allows families to plan their lives without worrying about appointment availability, and allows our clinicians to have stable work schedules and strong work-life balance.Fill-In Session Option
For families on our waitlist, we offer the option to schedule sessions on a fill-in basis based on appointment availability. While these sessions may be less consistent than the regular schedule, it allows kids to receive clinical support until a slot opens up on our Regular Session Schedule.We understand that responsibilities change across time, so there is the option to change your child’s weekly schedule, given ample notice. We reserve the right to decline requests for schedule changes based on availability, adherence to related NPT attendance policies/procedures, and/or frequency of requests.

Attendance and Cancellations
NPT understands that scheduling can be complex and, at times, hard to navigate. We take every possible step to ensure that appointment scheduling can go as smoothly as possible for the families we work with and our practitioners. We want to continue serving our community with the highest quality of care, so the policies below have been put in place for all families who receive clinical services through our practice.
These policies are included in the intake paperwork completed when a family establishes care with us. An authorized caregiver for every client (child) is required to sign these policies before their child can participate in therapeutic services at NPT.

Telehealth
Telehealth is available for evaluations, therapy sessions, and other clinical services that NPT provides.
Our telehealth options allow your child’s clinical support to continue in situations like mild illness, inclement weather, or other circumstances that take away the option of in-person visits. Given sufficient notice as outlined in our Attendance and Cancellation Policy, we have the option of full or as-needed telehealth services, as well as a hybrid option for clients who have therapy sessions more than once per week.
By participating in telehealth services at NPT, a client’s legal guardian is agreeing to the following policy:

Collaboration With Other Professionals
We believe that there is a lot of value in collaborating with other professionals. If you would like us to connect with another professional who works with your child, please let your clinician know! Then, we can work together to support your child’s growth.
Once your child becomes a client at NPT, we can only share protected health information with the expressed consent of their legal caregiver through a Release of Information form. NPT has their own version, linked below, or the other professional can have one signed on their end and sent to us.
